The Department of Health and Family Welfare of West Bengal is providing Birth Certificates Application Facility to the citizens of the State Through Janma-Mrityu Tathya Portal. Those people who want to apply for the Birth Certificate in West Bengal can Apply Online through Citizen Services on e-District WB Portal or Janma-Mrityu Tathya Portal.
WB Birth Certificate Application Will require information of both the Father and the Mother and Data of the baby like Weight, Time, Discharge Certificate, and Duration of Pregnancy.

WB Birth Certificate Apply Online Process
- Visit the Official Website of Janma-Mrityu Tathya Portal.
- Click on the Citizen Services and Select Apply for New Registration.
- Now Enter Your Mobile Number and Get Verified by OTP.
- Now complete the Registration form with child information, parent’s Information, and Address.
- Upload the Required ID Card of both Mother and Father.
- Choose the Type of Hospital and Enter the Permanent Residence Address.
- Click on the Submit Button given on the Screen to complete the Application.
WB Birth Certificate Track Application
- Visit the Official Website of Janma-Mrityu Tathya Portal.
- User will have to click on “Track Application” from the Birth sub menu of the Citizen Services main menu
- A popup will open where the citizen must enter the acknowledgement number.
- Citizens will enter the acknowledgement number and press the submit button.
- Clicking on the submit button, the current status of the application will be visible for citizens.
WB Birth Certificate Download Process
- Visit the Official Website of Janma-Mrityu Tathya Portal.
- Users will click on “Download Certificate” from the Birth sub menu of Citizen Services main menu for downloading certificates.
- A popup will open where citizens have to enter the acknowledgement number.
- Citizens will enter the acknowledgement number and press the submit button.
- Download link will appear for downloading the required birth certificate.
